We provide a coordinated approach to monitor health and well-being for people under 65 who have dementia or at risk of dementia. We also provide support for their carers and family. The program is designed to facilitate social participation independence and quality of life. Through a holistic person centred approach this is achieved through early intervention. The program will also support community involvement and social interaction.
The service will provide a social program within a café style environment in partnership with Terrigal Uniting Community Centre.
The aim is a coordinated individual focus in regards to social and lifestyle support. This helps maintain a sense of belonging and wellbeing for all involved.

Do I need a referral?
Referrals can be made by anyone including yourself.
What types of support can be provided?
• Social program in a café style environment • Guest speakers on topics of interest • Health and wellbeing monitoring • Life planning support • Leisure and learning activities • Family/carer support including support for children
What are the costs?
An affordable fee for the service applies which can be negotiated with the coordinator. People who may feel financially disadvantaged are not excluded from receiving services, and should discuss this matter with the Coordinator.
Can I have an advocate?
An advocate of your choice is welcome at all meetings. If you require an advocate, we can recommend an advocacy service or will contact the advocate/advocacy service of your choice.
