Home and Community Care aims to provide a comprehensive, coordinated and integrated range of basic maintenance and support services for frail aged people, people with a disability and their carers.We also provide support for these people to be more independent at home and in the community. This helps to enhance quality of life, and can prevent inappropriate admission to long term residential care.

  • Eligibility to HACC services is not automatic, and is allocated to those with the greatest need
  • You do not have to provide evidence of residency in order to receive a service
  • The waiting time can vary for different services
  • HACC services are provided as a low cost service
