Referral and Assessment The Client Information and Referral Protocol and the Comprehensive Assessment Protocol were both last reviewed in 2002. Since that time, there have been some minor modifications in both areas. As a consequence of these changes, the Central Coast Home & Community Care Development Advisory Committee provided information indicating that the two Protocols should be combined and information on Functional Screening should be included. A flowchart was also recommended.
At the same time as this review, the Commonwealth Department of Health & Ageing is reforming the Community Care Program. One aspect of that review is ensuring that clients are able to gain access to the services offered through one access point. This protocol endeavours to bring all processes together under a new project – The Central Coast Care Communication Network.

Abuse Protocol Abuse of vulnerable older people, adults and/or younger people with disabilities and their carers is a human rights issue. There are particular processes that need to be followed when situations of suspected or actual abuse are encountered. This document incorporates these processes when agencies/services providers encounter suspected or actual abuse.
Vulnerable older people (including those with a cognitive impairment, physical disability, sensory impairment or social isolation) and young people/adults with disabilities (including mental illness, physical disability, intellectual disability and acquired brain injury) are considered at risk, particularly if they are dependent on others for their care.
Carers are also considered victims of abuse if they experience wilful or unintentional harm caused by the person for whom they are caring.
All of these people listed above have a right to competent professional assistance, and the right to make decisions about which interventions, if any, they accept. This Protocol has been designed to provide information relevant to situations of abuse and covers clients who:
- Live at home;
- Are over sixteen years of age; and
- Are frail, or have dementia, or a physical, intellectual, sensory or psychiatric disability or brain injury.
- And their carers.
